Academic Visit Program Report, 10th-14th of December 2012

Monday, 10th of December 2012
Place/Departments: Director Office Room and Dept. of Laboratory Science.
Today activity was started with introduction and asking for permission from the Director of Gunma University Hospital, Prof. Nojima. The delegations think that he was really friendly and give us some message to enjoy this program as good as we can. Then, we discussed about Japan’s health problem. In fact, the patient that come to the hospital mostly because of eye problem, especially degeneration induced-eye problem.



He also recommended us to visit to Dept. of Radiooncology so that we can get the explanation more about the heavy ion method that used for cancer treatment,the third in the world. After that, we taking a picture together before leaving, and say our gratitude of to him.

Then, the first department visit was in Dept. of Clinical Laboratory . We visited the outpatient and inpatient clinic. In the inpatient clinic,  we saw some nurses and laborarian was doing phlebotomy to the patients to get the blood sample, then after that we saw the transportation system machine with it’s own tract in the ceiling and that’s all done automatically. It went to the laboratory from procedural room and vice versa.

Then we went to the laboratory that divided to 6 division ; the hematological division, microbiological division, hormone test division,  etcetera. After the specimen arrive at each division, the laborarian then take it and place the speciemn in the machine, then the machine do the test. For the double checking, the laborarian also done the test manually to confirm. Then the data admit to the computer and server, and may be check by all of the computer in the hospital. For the specimen, each slide have it’s own barcode, so the specimen would not be replaced with other specimen. For the maintenance of the machine, once every months the technician check it. How great regulation system!

Then the head of Anesthesiology Department, Prof.Saito, took us to visit Pocari Sweat Factory. Actually, Prof Saito planned to taking us to the Defibrillator Factory that Gunma Prefecture famous for. But because of some reasons, we might not visit the factory. So then he took us to The Pocari Sweat Factory. We get the explanation about the process of making the products and the benefit of the products. We also entered their museum to see their period to period of Pocari Sweat production in several countries, including Indonesia. After that, the academic visit for the first day is over and at 18.30 we start the welcoming party.

Tuesday, 11th of December 2012
Place/Departments: Dept. of Internal Medicine, Dept. of Anatomy.
In Department of Internal Medicine, we met Prof.Kurabayashi, the head of the department from the Cardiology division. At first, we introduce ourselves and told prof Kurabayashi about the meaning of our name. Then we get lecture from Prof.Kurabayashi about ECG. The teaching method is attractive and focusing on the understanding of the main basic concept. Involving students to also think and participate, and the professor really apreciate the students thought. Never pointing and told the students wrong, but teach wisely and the students realise by themself of their mistake and knowing the truth nicely.


Then we visited inpatient ward, where the professor gave us several quiz and explanation. After that we visited the examination room, we saw the ECG, Automatic Blood Pressure etcetera. Then after that, the visit is done, we also gave the professor souvenir, Gambang and then we took picture.

Then, in The Department of Anatomy, we accompanied by assistant professor, Murakami-sensei. In the laboratory, we saw each group of students that consist of 4 students are learn from one cadaver that use for 3 months of anatomy class. Each cadaver have been scanned by CT Scan, and the information have been added to the server. Each group given one iPad. The group may access the CT Scan of their cadaver from the iPad, that with some application, they may saw the 3 dimensional picture of the cadaver from the skin, fascia, muscle, bone, even the vascularization, innervation and lymphatics. Then they compare it with the real body of the cadaver. The departement combine the conventional methods and the modern method.

After 3 months of anatomy class. Then the cadaver is cremated and buried. The funeral were held by the university and all students that attend the anatomy class also attend the funeral. The education process to implement the value of appreciation and reciprocation. Then after finish the visit, we giving the souvenir for the departement and not to forget taking picture together.

Wednesday, 12th of December 2012
Place/Departments: Gunma Prefectural Office, The Retirement Home
After we visited Gunma Prefectural Office and had a lunch with Prof. Koyama, we continued our activity to visit the Retirement Home which belongs to doctor Takao Nakura. The place has some buildings. The biggest one has 3 floors. The place was built 15 years ago. It has inpatient service and outpatient service. In Japan, which has high life expectancy, the number of elderly population is high. Unfortunately, because the children have to work, so the children cannot attend the whole time for look after their parents. The position of the every person is much respected in Japan. Every person respected well even though they have different in come or salary. So, there is no servant in Japan.  This kind of retirements home helped the children very much. Doctor Nakura organized his retirement home with “Homely Environment” Principe. It is aimed for the elderly feel this kind of place is like their home. This retirement home never drives out for the family who couldn’t pay. It is because it has been the social norm of this retirement for taking care the elderly even though the family couldn’t pay. The service of the retirement was done by the modern way but still using happy method with the well skilled nurses. The fee is about 110.000-120.000 yen per month.

            Our activities in the retirement home were very commotion and inspiring. The elderly was very spirit and happy for welcoming us. Even the elderly studied and prepared some of the words and sentences in Bahasa (Indonesian language) for greeting us. They presented a song for welcoming us. We also presented a song for them. Doraemon  song  in Bahasa. We also tasted some of the food which made by them. It was nice food. They also made some of the handmade souvenirs for us. It was very touching moment. We were directly remembered to our grandmothers and grandfathers. Some of the elderly cried and smile happily. We did it too.

Place/Departments: Dept. of Physiology, Student Presentation
Our activity began in Department of Physiology at 9 a.m. We had an honor to get explanation about tsunami issue and its impact to health from Prof. Koibuchi. It was so interesting topic for us because one of city in Indonesia also was destroyed by tsunami in 2006, and ,of course, diseases including PTSD, came out because of natural disaster. We were surprised that the city in Japan which susceptible for earthquake is nuclear center. Because of that, when there was earthquake, the impact was huge, such as increased endocrine disease and the risk of cancer. Besides, the foods from there was contaminated, so the economic development was interfered.

After the lecture, we went to another rooms to saw activity of this department. There was dr. Roni, our senior, guided us and explain the research activity in this department. We saw rats which will be research subjects, and, of course, sophisticated microscope, centrifuge machine etcetera.

In the afternoon, we had discussion time which was started at 16.00. Gunma delegates explained us about health issue in Japan and education program in Medical Faculty of Gundai. The presentation gave us a lot of information, such as the serious health problem in Japan, cancer and aging. Infectious diseases is not a problem, not like in Indonesia, because the people do aware about hygiene and can prevent transmission. After that, it’s time for our turn to give explanation about health issue of Indonesia, and national insurance system.

Although there weren’t so many students who came to the discussion, because they had to prepare for exam in the next day, we were still enthusiastic to promote Indonesia through the video we presented. Issue about insurance system that will be held in Indonesia in 2014 was interesting too, because in Japan actually the system is similar. Precentage of the coverage is 70%, and the citizen need to pay 30% of the cost if they got sick.

After that, in the afternoon we had a  farewell party with professors, lecturers and students in student centre of Gunma University.


Friday, 14th of December 2012
Place/Departments: Dept. of Neurosurgery and Dept. of Radiooncology

The Department of Neurosurgery allowed us to visit the surgery room. After we changed our surgery dressing, we went upstairs to the surgery room and met doctor Hirato and Tosaka. The neurosurgery team were doing decompression surgery of a man that has a neuropathy because of the compression of the cervical vertebrae. The surgery has proceed about 2 hours and expected to finished about 3 hours later. They were opening slice by slice of the patient’s tissue to reach the compressed vertebrae, after they shifted laterally the esophagus, trachea and other vital artery, nerve and vein. Everyone just so interested and amazed with the neurosurgery team.


            Beside that, we allowed to show the other room that were doing surgery for a man that has an occlution of the basillary artery. We used protection vest also for prevent the radiation. We saw how they enter the surgery kit to the artery from monitor of operation room, it was exploded so that the vessel can be opened and blood flow well again.

The last department was The Department of Radiooncology. This department provide the sophisticated medicine technology for cancer treatment with the best precision nowaday, that mostly use particle carbon. Before we allowed to sightseeing around the facilities of this department, we got a lecture about radiology principle and method from Prof. Ken Ando. We also get a lecture about the comparison between heavy ion with another cancer treatment method such as using proton.


After lecture, we walked around to see the facilities for radiological examination, from X-Ray, CT Scan, MRI, until finally we saw Heavy Ion machine itself that in research progress to developed so that it can treat cancer slice per slice as scanning movement. How amazed we are!

The department visit was over today. And in the afternoon we had a pasta party with Department of Public Health as a farewell party also because today was our last day in Gunma Prefecture on that time.

It was really unforgettable moment. We hope we may see each other again, may destiny meet us all. And, the last but not least,
